Membership Application Form

Apply online for membership of Motijheel Computer Society.

Required documents

a. Attested copy of a current trade licence — the nature of business must mention computers and/or ICT, and the address on the application must match the address on the trade licence, failing which the trade licence address must be attested.
b. For a new taxpayer, an attested copy of the TIN certificate; for an existing taxpayer, a copy of the current income tax payment receipt — of the proprietor for a sole proprietorship, of the company for a partnership or limited company.
c. A company profile describing the organization's capacity and experience.
d. For an incorporated organization, an attested copy of the Memorandum & Articles of Association; for a partnership, the Deed of Partnership — the nature of business must mention computers and/or ICT.
e. A current bank solvency certificate (original) — in the proprietor's name for a sole proprietorship, in the company's name for a partnership or limited company.
f. Two recent colour passport-size photographs of the representative, one visiting card, and a copy of the NID card.
g. Attested copy of the VAT registration certificate (if held).
h. A PO/DD for ৳ 25,000 in favour of Motijheel Computer Society.

Points to note

MCS membership is processed in accordance with the DTO Rules and the Articles of MCS.
Only organizations engaged in the computer and/or ICT trade are eligible for MCS membership.
The organization's registered address must be attested, and MCS reserves the right to verify it.
MCS reserves the right to ask for any document beyond those listed, including originals.
MCS reserves the right to inspect the applicant's registered and/or business address in person; membership is approved only if that inspection is satisfactory.
MCS may call the organization's representative for an interview if needed.
Organizations in Khulna, Sylhet, Jessore, Rajshahi, Barisal, Chattogram, Cumilla and Mymensingh must apply through the relevant MCS branch committee.
Incomplete applications are not accepted.
Nothing confirms MCS membership until verification and attestation are complete.
Under clause 7 of the MCS Memorandum & Articles of Association, the MCS Executive Committee alone has the authority to approve or decline any membership application.
